Social Worker - Urgent Position

Reports to the Manager for Development & Learning Support with the following role & responsibilities:

1. Case Management

• Collates all information on recommendation of the child for intervention education, including if required conduct bio-psycho-social assessment of the family and child needs, risks and strengths through participation in case discussion.
• Participate as a member of the team in Case Filter meetings with the mutli-disciplinary team from various organizations in the review and recommendation of the intervention plan for the child.
• Conduct social work-parents meeting to share assessment outcomes and recommendations with the parents.
• Facilitate parents' understanding of programme and needs of child to obtain consent from the parents, including conducting follow up engagements.
• Provide necessary family information to support any financial assistance required resulting from means testing.
• Responsible for accurate case notes for communication for all parties.
• Develop case conceptualization and overall plans and follow ups regarding goals and outcome for the family and child including spearheading multi-disciplinary team discussion not limited to educators, parents and Learning & Development Support team.
• Engage and work collaboratively with external social service agencies for holistic case management for the families.
• Identify resources and other institutions/agencies available, including making referrals for the support of the family that is required for the family and child to ensure child’s prompt attendance in the classroom and intervention plans to achieve optimum development of the child.

2. Documentation and Administrative tasks

• Participate in Case Filter Meetings and maintain good record keeping as it will be required for the social-work-parents meeting.
• Complete documentation (e.g. case notes) accurately and promptly for other professionals who are working with child.
• Maintain accurate records, prepare reports for governing agencies, collate information and submit government means-test applications.

3. Collaborative Partnerships/Community Network

• Demonstrate knowledge of available community resources.
• Engage and work collaboratively with various community agencies (e.g Family Service Centres, Social Services Offices or other agencies).

Job Requirements

• Possess a Diploma/ Degree in Social Work or its equivalent with minimum 2 years experience within social community setting /pre-school with understanding of working with families and young children.
• For degree with social work, to be registered with Singapore Association of Social Workers (SASW), preferably a member.
• Additional qualification in Early Childhood and/ or Child Psychology will be an advantage.
• Possess exceptional written and spoken skills in English with high competency in software applications & presentation skills.
• A multilingual all rounded people person with excellent interpersonal and communication skills.
• Meticulous and have good administrative and communication skills.
• Able to manage multi cases.
• Parent engagement skills in communication and counselling with patience and resilience.
