Wellbeing Coordinator, Integrated Seniors Health and Wellness (ISHW) Hub - Urgent Hire

The Job:

The roles and responsibilities of the Integrated Seniors Health and Wellness (ISHW) Hub Wellbeing Coordinator (WBC) will include but not be limited to the following:

• Support ISHW programme team in implementing and operationalizing activities to achieve key objectives.
• Engagement of residents in the community to enroll them under ISHW hub, which include activities such as health screening, outreach events, talks and community activities
• Provide integrated person-centred health and social care to residents enrolled under the ISHW through care coordination work and working with clinical teams to ensure care plans are in place for residents and communicate the care plans to residents
• Engage and liaise with internal and external stakeholders, including clinical and operational departments from SingHealth institutions, external community partners such as eldercare service providers, primary care partners and government agencies involved in the programme where necessary
• Monitor programme progress and provide regular updates to programme director, senior management and relevant agencies/funder. 
• Maintain necessary documentation for audits 

Job Requirements:

• Bachelor Degree in any discipline, preferably in healthcare, nursing or sociology discipline
• Operational experience in and knowledge of health social and community care sectors preferred
• Good verbal and written communication skills
• Versatile to adopt a variety of coaching methods, as suited for different resident and patient profiles
