Agape Plus is looking for Caregivers / Healthcare Assistants / Patient Care Assistants who are compassionate, reliable and committed to supporting individuals and families who require care in the community.

Depending on your availability and commitment, we welcome applicants who are keen to join us on a freelance, short-term contract or full-time basis.

Key Responsibilities

• Provide general caregiving and personal care support to clients
• Assist clients with activities of daily living such as feeding, toileting, showering, grooming, dressing and mobility
• Provide companionship and emotional support to clients, including seniors who may feel lonely or require regular supervision
• Support clients with safe transfer, positioning and movement, including from bed to wheelchair, wheelchair to chair, or during walking assistance
• Assist clients with basic exercises, mobility routines or rehabilitation activities as instructed by the family, therapist or care supervisor
• Provide care support for stroke patients, including mobility assistance, transfer support, feeding assistance, personal hygiene care and close monitoring of their general condition
• Provide care support for dementia patients, including supervision, companionship, reassurance, routine-based care and support in managing confusion, restlessness or behavioural changes
• Support clients who require fall prevention supervision and ensure that their surroundings are kept safe and clutter-free
• Provide medical escort support for clients attending hospital appointments, polyclinic visits, therapy sessions or medical reviews
• Assist clients during medical appointments by accompanying them safely, helping with registration, waiting with them and updating the family or supervisor where required
• Monitor the client’s general wellbeing and report any unusual changes, concerns, discomfort, pain, mood changes or incidents promptly
• Assist with meal preparation, feeding support and hydration reminders where required
• Provide medication reminders where instructed, without administering medication unless authorised and trained to do so
• Support with personal hygiene care, including continence care, diaper changing and maintaining cleanliness and dignity
• Maintain a clean, safe and comfortable environment for the client
• Assist with light housekeeping related to the client’s care area, such as changing bedsheets, cleaning up after meals and keeping care items organised
• Follow the agreed care plan and instructions given by the supervisor, family or healthcare professionals
• Communicate clearly and professionally with clients, family members, supervisors and care coordinators
• Maintain client confidentiality and respect the client’s dignity, privacy, culture and personal preferences
• Record and update basic care notes, observations or incident reports where required
• Respond calmly and responsibly during emergencies and escalate matters immediately to the family, supervisor or emergency services where necessary

Requirements

• Prior caregiving, healthcare or patient care experience will be an advantage
• Applicants without experience may be considered if they have the right attitude and willingness to learn
• Experience supporting elderly clients, stroke patients or dementia patients will be an added advantage
• Must be patient, responsible, respectful and compassionate
• Must be willing to attend training or briefing sessions arranged by Agape Plus
• Able to commit to assigned shifts based on agreed schedule
