Homecare Staff Nurse ( Locum , Part-Time)

As our home care services continue to grow, we are looking for dedicated and passionate nurses to join our team in delivering exceptional community-based healthcare across Singapore.

JOB SCOPES :

Home Nursing Care Plan:

• Assess general health condition of clients and document in care plan.
• Evaluate social history, including financial constraints; make referrals/recommendations.
• Assess home environment; provide necessary referrals/recommendations.
• Advise clients/caregivers on care plans for continuity of care.
• Anticipate and resolve issues; refer to supervisor when needed.
• Document visit observations, procedures, and action plans within one workday post-visit.
• Import and upload wound images and vital signs as needed.

Nursing Procedures:

• Wound dressing and management.
• Change and manage indwelling urinary catheters (IDC).
• Change and manage nasogastric feeding tubes (NGT).
• Manage stomas and their appliances.
• Medication packing and reconciliation.
• Flush PICC lines and change site dressings.
• Provide oral and tracheostomy care and suctioning.
• Conduct customized caregiver training.

Job Requirements:

• Registered with Singapore Nursing Board with valid certification.
• Minimum 2 years of nursing experience in a medical or geriatric ward/community setting within the last 3 years.
• Experience in oncology or palliative care is an advantage.
• Strong communication and interpersonal skills; comfortable with diverse individuals.
• Positive attitude towards learning and feedback.
• Adaptable and collaborative team player.
• Independent and reliable; performs well in all aspects of work.
• Ability to make sound clinical judgments in homecare settings.
• Keen interest in supporting end-of-life care for homebound seniors.
