Job Responsibilities

• Patient Care Management: Taking comprehensive histories, performing physical examinations, executing clinical procedures, and completing follow-up reviews.

• Patient & Caregiver Education: Communicating treatment plans, conducting discharge teaching, and training families on care.

• Clinical Supervision: Guiding Junior Enrolled Nurses, Nursing Aides, and Care Associates.

• Infection Control & Documentation: Maintaining clinical records and upholding standard precautions during home visits

Requirements:

• Valid SNB and BCLS

• Able to commit at minimally 3-5 days a week, 8:30am-6pm

• Home care/nursing home experience required OR interRAI certified

• Have taxi card/grab account for transportation
