Health Coach (Adjunct) - Part-Time

As a Health Coach, you'll play a vital role in helping individuals achieve their health and wellness goals through tailored guidance and support. You will work with participants both individually and in groups, focusing on education, motivation, and accountability to facilitate positive lifestyle changes.

Job Description:

• Conduct assessments to evaluate a participant's health status, including medical history, lifestyle habits, and current health conditions.
• Conduct health coaching sessions on guided stipulated health topics.
• Develop personalized wellness plans that include nutrition, exercise, stress management, and other lifestyle modifications.
• Support necessary administrative duties such as documentation of coaching notes.

Job Requirements:

• Minimum Diploma in Nursing, Nutrition and Health Promotion related disciplines
• Good knowledge in health promotion topics
• At least 3 years of coaching or patient education experience would be a bonus
• Ability to communicate in English and Mandarin/Malay/Tamil and/or also preferably a dialect to cater to diverse clientele
• Patient and personable
